Iodine deficiency can lead to an enlarged thyroid gland due to the thyroid developing a goiter in an attempt to try and keep up with thyroid hormone production. Since the thyroid utilizes iodine in order to produce active thyroid hormone T3, if insufficient iodine levels are present in the body then this will cause hypothyroidism. The pituitary gland in the brain will recognize that the thyroid hormone level is too low and send a signal to the thyroid to produce more hormone; the signal is called TSH or thyroid stimulating hormone. Just as it sounds this hormone stimulates the thyroid and also makes it grow in size. The unusual growth of the thyroid is what is called a goiter.

The absence of sufficient iodine concentration can lead to a lack of energy. When sufficient levels of iodine are not present the thyroid cannot convert the inactive thyroid hormone T4 into the active thyroid hormone T3. The active thyroid hormone T3 is what is used to regulate your metabolism and carry out biochemical reactions that are related to producing the energy molecule known as adenosine triphosphate (ATP). This energy molecule is used in every biochemical reaction that occurs in the body. Thus, an increase in thyroid stimulating hormone (TSH) and a decrease in T3 can lead to a lack of energy and also a diagnoses of a form of hypothyroidism.
